For some Paramount Elite plans, Paramount Elite will cover up to 24 one-way trips to doctor appointments, pharmacies and other medical-related activities. 

Trips are limited to 35 miles, one way. All trips must take place in our Paramount Elite service area, which includes Fulton, Lucas, Ottawa, Sandusky and Wood counties in Ohio and Branch, Hillsdale, Lenawee, Monroe and Washtenaw counties in Michigan. Trips must be scheduled at least three business days in advance.

In-home Health Assessments

This annual visit is a convenient way to get important preventive care, complementing but not replacing the care you get from your primary care provider. A clinician from our trusted vendor Porter can visit you at home or complete a virtual visit.

Telehealth Services

Telehealth connects you with a licensed provider through real-time audio and video, using your smartphone, computer or tablet. It’s a convenient option for getting healthcare from the comfort of your home.  

When you see an in-network provider for a telehealth visit, you’ll pay the same copay as you would for an in-person provider visit.

Worldwide Coverage 

No matter where your adventures take you, your Paramount Elite Medicare plan goes with you. The coverage includes limited copays for emergency care, urgent care and emergency ambulance services outside of the United States. Plans have a combined maximum allowance each year for emergency coverage outside of the United States. 

Keeping You Safe

Personal Emergency Response Service (PERS)

Members on certain Paramount Elite plans have access to a medical alert system through NationsBenefits^® in partnership with Connect America^®. This benefit allows members to receive around-the-clock monitoring for greater independence and safety. Your medical alert system provides you with help at the push of a button. If your plan includes this benefit, refer to your Evidence of Coverage to learn about the different PERS devices available.  

Bathroom Safety and Fall Prevention Devices 

Members have a maximum coverage amount of $100 per calendar year for purchasing plan-approved bathroom safety devices from the NationsBenefits catalog. This includes items such as bath safety benches, gait belts, handheld shower heads, chrome grab bars, raised toilet seats, toilet safety rails, transfer benches and tub safety bars. Supporting Your Health and Wellness 

Home Meals Program

We provide our Medicare members two meals a day for seven days after being discharged from an inpatient hospital – at no cost to you. 

SilverSneakers^® 

All Paramount Elite members get a SilverSneakers^® fitness membership at no additional cost, with access to thousands of fitness centers across the country. In addition to fitness centers, the program includes cardio classes, access to pools, yoga and virtual fitness classes. 

Wellness Reward

All Paramount Elite plans include a wellness reward. When you complete certain important preventive care services, you’ll earn $50.

$0 Annual Physical Exams 

Unlike many Medicare plans, Paramount Elite covers an annual physical exam with your primary care provider at a $0 copay. Paramount Elite also covers the Medicare annual wellness visit at no cost. While an annual wellness visit is very important, it does not include a physical exam component. We’re proud to offer full coverage for both of these important visits.
